Introduction

This is the implementation guide for the e-card Profile based on HL7® FHIR® R5.

E-card FHIR Profile is a FHIR representation of an Austrian social insurance plan. It is built on top of the Austrian Core Profile as it derivation.

E-card Profile is hosted on the Vdas-on-Fhir. The most recent version of this implementation guide can be found at the www.chipkarte.at.

Core Profile is hosted on the HL7® Austria github project. The most recent version of this implementation guide can be found at the HL7® Austria FHIR® Website.

This implementation guide is provided to support the use of FHIR® in the e-card services in Austria.

This guide is a working specification. We expect it to be implemented and tested by FHIR® system vendors, whose feedback will help improve its content. With this IG for Trial Use, we are seeking feedback on whether the following objectives have been met:

  • The guide provides guidance on the e-card VDAS Proof of Concept including the e-card Profile on HL7 FHIR Coverage.
    • Along with the Coverage profile the Proof of Concept uses the HL7 Austrian Patient - a profile on the patient resource from the core Profile.
  • This guide provides a set of sample Code Systems and ValueSets based on FSH files.

Proof of Concept: VDAS on FHIR

This implementation guide should support healthcare software providers to try out the e-card service VDAS (Versicherungsdatenabfrageservice) API with the use of FHIR. We used the Austrian patient from the AT Core Patient Profile, created a profile on Coverage and defined ValueSets and CodeSystems accordingly. Please note that there will be a mixture of German and English vocabulary due to the fact, that HL7 FHIR uses English terms and the e-card system mainly uses German terms.

VDAS

The Versichertendatenabfrageservice (VDAS) enables hospitals and other healthcare providers to query the insured person's data online via the e-card infrastructure. This query can be for a current date or for a key date in the past.

As a result, the insured person data (claim and, if applicable, derived claim or derived claims, or only the patient data) of the social insured person found is returned, subject to the following technical conditions, which are guaranteed by the web interface or the contractual partner's software:

  1. In principle, the social insured person (SV) has the right to choose which sickness insurance scheme (SHI) to use and is able to provide information about it.
  2. If the insured person is not able to disclose or choose the claim, the health insurance claims are determined at a later date (when the insured person is able to provide information again).
  3. Each request must be signed, either with a social insurance signature card token (with e-card or admin card) or with a software certificate. Please note that the admin card or software certificate must match the admin card or software certificate of the application. It is not possible to set up a dialogue with an admin card but to sign the VDAS request with a software certificate and vice versa."

HL7® Austria is an official Affiliate of HL7® International. Within HL7® Austria the technical committee for FHIR® (TC FHIR®) is responsible to promote and disseminate the standard HL7® FHIR®. The TC FHIR® deals with the standard-compliant and coordinated usage of HL7® FHIR® based communication solutions. It coordinates and describes necessary localizations and offers concrete help for FHIR®-compliant interfaces.
